Contest Instructions
How it works:
1- Every month, Hockey Supremacy chooses 1 stick from its inventory, and commits to giving it away.
2- Contestants are asked to fill in this form, thus providing the description of their dream hockey stick (brand, model, curve, flex). N.B. Only apply once, or you entry will be void.
3- The first person to submit a form, which contains the exact model (brand, model, curve and flex) of the stick we have chosen for the month, will be awarded the stick.
Prize Eligibility
Only persons residing in Canada who are at least 13 years of age can enter.
