Individuals may enter this Sweepstakes by filling out the online form only once. More than one entry from any person or e-mail address will void all entries from that person or e-mail address.
Entrant will be disqualified if the entry contains profanity, copyrighted material, lewd or lascivious behavior, or is deemed offensive. Entry must be approximately 60 seconds in length. Please try to stay as close to 60 seconds as possible.
By ENTERING the sweepstakes, the entrant agrees that the Sponsors and those acting under the Sponsor’s authority may use entrant’s name, picture/portrait likeness and/or voice, for advertising and promotional purposes without further consideration, unless prohibited by law.
Drawing:
This Sweepstakes will have one (1) drawing and one (1) winner who will receive Gentle LASIK at Cutarelli Vision in Fort Collins, CO. Winners must comply with all terms and conditions set forth in these Official Rules, and winning is contingent upon fulfilling all such requirements. Finalists will be chosen by Cutarelli Vision if necessary. Finalists will only be chosen if there are more than 30 entries. Winner will be chosen by Facebook Fans.
The entrant with the highest number of votes from Facebook Fans of Cutarelli Vision will receive Gentle LASIK. Voting will open the last week beginning 8/29/11 at 10:00 am MDT and closes on, 9/5/11 at 5:00 pm MDT All entries MUST be received by 8/28/11 by 8/28/11 at 11:59pm MDT. Odds of winning depend on the number of eligible entries received.
Potential winner will be notified by Cutarelli Vision on or around 9/5/11. If a potential winner cannot be contacted within fourteen (14) days after the first
attempt to contact such potential winner, an alternate entrant will be selected in his or her place at random from all entries received.
