Eligibility: The ACUVUE® Brand "1•DAY Starts Today Teen Video Contest" (the "Contest") is open and offered only to legal residents of the 50 United States and District of Columbia and Canada (excluding residents of Quebec) who are between 13 and 21 years of age at the time of entry. Employees, officers and directors of Johnson & Johnson Vision Care, Inc. ("Sponsor"), and its parent, subsidiaries and affiliated companies, advertising, Contest or production agencies, and their immediate family members (spouse, and parent, child, or sibling and their respective spouses, regardless of where they reside) individuals residing in their same household, whether or not related, are not eligible to participate, enter or win. Professional actors, as determined by Sponsor, are also not eligible. If entrant is an eligible minor, he/she should have her/his parent’s or legal guardian’s permission to participate. Sponsor has provided on the Web site a selection of music that you may choose to use in your Submission ("Sponsor Music"). Please note that he Sponsor Music may only be used in conjunction with your Submission into this Contest. All other usage of the Sponsor Music is unauthorized and constitutes a violation of international copyright laws and can subject you to fines and criminal prosecution.
As part of your Submission, and for the purposes of determining your eligibility for different Entry Periods (as described below) you will be asked to select one of the following based on which one best describes your current vision care: 1. I wear contacts. 2. I want contacts. 3. Don’t need/want them. Wearing or not wearing ACUVUE® contacts will not impact judging.

Limit one (1) Submission per person/email address for the entire Contest Period. Any attempt by any person to obtain more than the stated number of entries by using multiple/different email accounts or any other methods will void all of that person’s entries and that person will be disqualified. Please note: You must have a valid YouTube or Google account and agree to the YouTube Terms of Service (https://www.youtube.com/terms), the YouTube Privacy Policy (https://www.youtube.com/privacy), and the Google Terms of Use in order to upload your Submission from YouTube. Registration on YouTube must be completed before a Submission can be submitted from your YouTube account. YouTube membership is free.
Submission Requirements/Restrictions: By submitting a Submission, each entrant (and, if an eligible minor, his/her parent or legal guardian) (i) warrants that it is an original Submission created by the entrant (with the exception of the Sponsor Music (if applicable)); and (ii) grants Sponsor and its designees the perpetual, unlimited, nonexclusive, transferable, worldwide and irrevocable right to modify, create derivative works of, edit, post, publish and/or otherwise use the Submission and all elements embodied therein, including any names and likenesses, in any manner and in any and all media now known or hereafter developed, without further compensation, notification or permission. To be eligible, a Submission must: (1) be entrant’s own original work that has not been submitted to any other third party, including any other contest (with the exception of the Sponsor Music (if applicable)); (2) be a .wmv, .mov, .avi, .mpg, .mpeg, .mp4, .asf, .3gp or a .3g2 format; (5) be owned by entrant; (6) be nonconfidential; (7) have been legally created; and ( if the video includes other individuals or was filmed by an individual other than entrant, they (or, if minors, their parents or legal guardians) must provide permission to the entrant to submit the Submission in the Contest. Submission may not contain, as determined by the Sponsor, in its sole discretion, any content that:
• promotes a Sponsor competitor or its products or services;
• promotes using the product in an offlabel manner or makes any mention of any adverse event related to Sponsor’s products;
• makes any claim about a product, Sponsor’s or otherwise, that cannot be substantiated;
• is sexually explicit, obscene or suggestive; unnecessarily violent or derogatory of any ethnic, racial, gender, religious, professional or age group; profane or pornographic; or contains nudity;
• promotes alcohol, illegal drugs, tobacco, firearms/weapons (or the use of any of the foregoing); promotes any activities that may appear unsafe or dangerous; promotes any particular political agenda or message;
• appears to duplicate any other submitted Entries;
• defames, misrepresents or contains disparaging remarks about other people or companies or Sponsor, its products or services;
• contains trademarks, logos, or trade dress (such as distinctive packaging or building exteriors/interiors) owned by others, without permission to the extent permission is necessary;
• contains any personal identification, such as license plate numbers, personal names, email addresses or street addresses or other personal information;
• contains copyrighted materials owned by others (including music, photographs, sculptures, paintings, and other works of art or images published on or in websites, television, movies or other media) (with the exception of the Sponsor Music), without permission to the extent permission is necessary
• contains materials embodying the names, likenesses, voices, or other indicia identifying any person, including, without limitation, celebrities and/or other public or private figures, living or dead, without permission to the extent permission is necessary;
• communicates messages or images inconsistent with the positive images and/or good will with which Sponsor wishes to associate; and/or
• violates any law.
By making a Submission to this Contest, an entrant (and, if an eligible minor, his/her parent or legal guardian) agrees that Sponsor may obtain other Submissions that are similar or identical in theme, format or other respects to that submitted by entrant. Entrant (and, if an eligible minor, his/her parent or legal guardian) acknowledges that, by making a Submission, Sponsor does not waive any rights to use similar or related ideas previously known to Sponsor, or developed by its employees, or obtained from sources other than entrant. By making a Submission, entrant (and, if an eligible minor, his/her parent or legal guardian) hereby grants permission for the Submission to be posted on the Web site or other Sponsor websites and otherwise used by Sponsor in accordance with the grant of rights set forth above. Entrant (and, if an eligible minor, his/her parent or legal guardian) agrees that Released Parties (as defined below) are not responsible for any unauthorized use of Submissions by third parties. Released Parties do not guarantee the posting of any Submission and may take down any Submission for any reason at any time. An entrant (and, if an eligible minor, his/her parent or legal guardian) agrees that he/she will not use the Submission for any other purpose, including, without limitation, posting the Submission to any other online social networks, without the express consent of Sponsor in each instance.
3. Winner Determination: Grand Prize Winner Determination: Approximately one (1) day following the end of each Entry Period, all eligible Submissions received by Sponsor for that Entry Period will be judged by a panel of qualified independent judges based on the following judging criteria and percentages: (a) fit to the Contest theme (25%); (b) uniqueness (25%); (c) quality of video (25%); and (d) passion in communication of the dream (25%) ("Judging Criteria"). The Submission with the highest score for each Entry Period, as determined by the judging panel based on the Judging criteria, will be deemed the potential Grand Prize winner for that Entry Period, subject to compliance with these Official Rules. In the event of a tie, the Submission with the highest score in the passion and communication of the dream criteria from among the tying Submissions will be deemed the potential Grand Prize winner for that Entry Period. If there is still a tie, the score received for fit to Contest theme, then uniqueness, and finally quality of video will be used to break the tie. The odds of winning a Grand Prize depend on the number of Submissions received in a given Entry Period, and the quality of an entrant’s Submission as compared to other Submissions submitted in the relevant Entry Period as determined by the judging panel based on the Judging Criteria.
First Prize Winner Determination: From 12:01 a.m. ET on May 23, 2011 through 11:59 p.m. ET on July 8, 2011, all eligible Submissions received by Sponsor for the entire Contest Period will be posted on the Web site where the public can vote for each Submission using the "thumbs up" or "thumbs down" functionality on the Web site. Please note that while someone can cast a thumbs down vote, such thumbs down votes will not be considered in determining the winner. The number of thumbs up and thumbs down votes a Submission receives will not be displayed on the Web site A Submission is not a winner until Sponsor declares a Submission the winner. Limit one (1) thumbs up or thumbs down vote per person/email address per Submission per day, but there is no limit to the number of Submissions a person can vote for. The one (1) Submission that receives the most valid thumbs up votes, as determined by Sponsor in its sole discretion, will be deemed the potential First Prize winner, subject to compliance with these Official Rules. Criteria (as defined below), and the Submission with the highest overall score from among those tied, as determined by Sponsor, will be deemed the potential First Prize winner. Entrants are prohibited from obtaining votes by any fraudulent or inappropriate means, including, without limitation, offering prizes or other inducements to members of the public, as determined by Sponsor in its sole discretion. Use of script, macro or any automated system to cast a vote or with the intent to impair the integrity of the voting process is prohibited and all such votes (both thumbs up and thumbs down votes) will be void. The odds of winning the First Prize depend on the popularity of an entrant’s Submission compared to other Submissions received during the Contest Period, as determined by the number of thumbs up votes received.
Sponsor reserves the right to not award all prizes for any prize level if, in its sole discretion, it does not receive a sufficient number of eligible and/or qualified Submissions for a prize level in a particular Entry Period or otherwise. All prize awards are subject to verification of eligibility and compliance with the terms of these Official Rules. The decisions of the Sponsor and judges are final and binding in all matters. Limit one (1) prize per person/household.
4. Grand Prize Winner Notification: Potential Grand Prize winners (or winner’s parent/legal guardian if winner is deemed a minor in the state/province/territory of his/her residence) will be notified by telephone, email or mail and required to complete, sign and return an Affidavit of Eligibility, a Liability Release, and Publicity Release (where legal) within 48 hours of date of notification. Noncompliance with this requirement, or if prize/prize notification is returned as nondeliverable, if a potential winner cannot be contacted within a reasonable time period or if a potential winner is found to be ineligible or not in compliance with these Official Rules, such prize will be forfeited and, at Sponsor’s discretion, may be awarded to a runnerup, time permitting. If a potential Grand Prize winner is deemed a minor in the state/province/territory of his/her residence, prize may be awarded in the name of his/her parent or legal guardian and he/she will be responsible for fulfilling all requirements imposed on winners set forth herein.
First Prize Winner Notification: Potential First Prize winner (or winner’s parent/legal guardian if winner is deemed a minor in the state/province/territory of his/her residence) will be notified by telephone, email or mail and may be required to complete, sign and return an Affidavit of Eligibility and Liability Release and Publicity Release (where legal) within two (2) days of date of notification. Noncompliance with this requirement, or if prize/prize notification is returned as nondeliverable, if potential winner cannot be contacted within a reasonable time period or if potential winner is found to be ineligible or not in compliance with these Official Rules, such prize will be forfeited and may be awarded to a runnerup. In addition, if the First Prize winner is a resident of Canada, he/she will be required to correctly answer a timelimited mathematical skill testing question or prize will be forfeited and, at Sponsor’s discretion, awarded to a runnerup. If First Prize winner is deemed a minor in the state of his/her residence, prize may be awarded in the name of his/her parent or legal guardian and he/she will be responsible for fulfilling all requirements imposed on winner set forth herein. Prizes & Approximate Retail Value’s ("ARV"): There is one (1) Grand Prize available to be won by eligible entrants (see rule 2 for Vision Care Categories eligible for each Entry Period) per Entry Period. Each Grand Prize consists of: A trip for two (2) to Los Angeles, CA, to meet and be mentored by a Sponsorspecified celebrity for a day on one of the dates specified in Section 2 above and a separate trip for two (2) to Los Angeles, CA to attend a een awards event in August 2011. Mentor Trip includes: roundtrip coach air transportation for two (2) from major commercial airport nearest winner’s residence to Los Angeles, CA, two (2) nights Sponsorspecified hotel accommodations (one (1) standard room, double occupancy); ground transportation to/from airport/hotel/meeting location in Los Angeles; opportunity for winner only (or, if a minor, with a chaperone) to be mentored for a day (up to 8 hours) with a Sponsorspecified celebrity, which will be filmed for potential future webisode or other use as determined by Sponsor in its sole discretion; and US$500.00 spending money, awarded in form of a check, for winner only; opportunity for winner only to be filmed for a possible television commercial and/or webisode (sometimes, collectively "Commercials"); and a oneyear supply for winner only of 1·DAY ACUVUE® MOIST® Brand Contact Lenses, awarded in the form of a US$460.00 Visa Gift Card, which can be used towards lenses, and an eye doctor appointment in Los Angeles (a minor winner must be accompanied by his/her parent or legal guardian). Teen awards event Trip includes: roundtrip coach air transportation for two (2) from the major commercial airport nearest winner’s residence to Los Angeles, CA, two (2) nights Sponsorspecified hotel accommodations (one (1) standard room, double occupancy); ground transportation to/from airport/hotel/een awards event in Los Angeles, and two (2) tickets to attend a 2011 een awards event in August, 2011. ARV of each Grand Prize Package: US$4,500.00 total (based on US national average departure costs). Any difference between the stated ARV and actual value will not be awarded. Exact details, including length of appearance in Commercials, prominence of appearance in Commercials will be determined by Sponsor in its sole discretion. Sponsor reserves the right to film, photograph and/or otherwise record the creation of the Commercials ("Creation Footage"). The Commercials and Creation Footage will be owned by Sponsor. Winners (and, if eligible minors, their parents or legal guardians) understand that Sponsor has the right to reproduce, distribute, perform, create derivative works of, edit, alter, display, exhibit, combine with other materials, exploit and otherwise use the Commercials and Creation Footage and/or all elements embodied therein, in any manner and in any and all media now known or hereafter devised throughout the Universe for any purpose whatsoever, including, without limitation, for commercial purposes, without compensation or notification to, or permission from, winners or any third party. Winners and guests must comply with all studio rules and regulations. Failure to do so may result in forfeiture of such portion of prize. In the event Grand Prize winner resides within 150 miles of Los Angeles, ground transportation will be awarded in lieu of air transportation and no compensation or substitution will be provided for difference in prize value. obligation. Any picture identification requirements, passports, visas, immunizations, or other requirements associated with travel are the winners’ and their guests' responsibility. All travel arrangements must be made through Sponsor’s agent and on a carrier of Sponsor’s choice. If winner is unable to travel on dates specified and agreed to, prize will be forfeited in its entirety and another winner may be chosen for the Entry Period at issue, at Sponsor’s discretion, time permitting. Released Parties are not responsible for any postponements or cancellations associated with any prize element. All costs and expenses associated with prize acceptance and use not specifically stated herein as being provided are the sole responsibility of the winner, including additional ground transportation, meals, gratuities and souvenirs. If guest is a minor, he/she must be the child or legal ward of the winner. Guest (or, if child or legal ward of winner, winner) must execute and return a liability/publicity release prior to issuance of travel documents. If winner is a minor, his/her guest must be a parent or legal guardian. There is one (1) First Prize available to be won during the Contest Period. The First Prize consists of two thousand dollars US, awarded in the form of a check. ARV of the First Prize: US$2,000.00. No substitution, cash redemption of noncash prizes or transfer of prize by a winner permitted. Sponsor reserves the right to substitute a prize (or portion thereof) of comparable or greater value, at its sole discretion. All federal, state, provincial and local taxes (including any income and withholding taxes) are the sole responsibility of the winners. 6. General Rules: Submissions, entries or votes generated by script, macro or other automated means or by any means which subvert the entry or voting process will be void. Sponsor, YouTube, and their respective parents, subsidiaries and affiliated companies, advertising, Contest or production agencies and all of their respective officers, directors, representatives, agents and employees (collectively, "Released Parties") are not responsible for lost, late, damaged, delayed, inaccurate, incomplete, garbled, mutilated or misdirected Submissions or entries or votes, or for Submissions or entries or votes not received by the applicable deadline, or for electronic, network or computer failures or malfunctions, lost or unavailable network connections, or failed, incomplete, garbled or delayed computer transmission that may limit a user’s ability to participate in this Contest, or other hardware, software or technical errors of any kind including any injury or damage to any person’s computer related to or resulting from participating in the Contest or for other errors or difficulties of any kind whether human, mechanical, electronic, computer, network, typographical, printing or otherwise relating to or in connection with the Contest, including, without limitation, errors or difficulties which may occur in connection with the administration of the Contest, the processing of entries or Submissions, the judging of Submissions, the tabulation of votes, the announcement of the prizes, or in any Contestrelated materials.
